New Account Form Fields

Required Fields

Required fields are identified by labels in Bold font. You will not be allowed to submit a request for a new account if any of the required fields are missing.

Name (required)

New user's real name. (First Last)

Phone

New user's phone number. A phone number where the new user can be reached during PDSF business hours (9:00 am - 5:00 pm PST) or where a voice mail message can be left.

Email address (required)

New user's email address.

FAX

New user's FAX number (area code) number.

Organization (required)

The new user's organization, experiment, division, or institution.

Nationality (required)

The new user's nationality. Required by NERSC.

Prefered Username

The new user's prefered Unix account name.

Prefered Shell

The new user's prefered default shell. Available shells include (not on all platforms):

• tcsh
• csh
• ksh
• bash
• sh

Accounts & Allocations (required)

These are the accounts which the new user is requesting. Possible accounts include:

• PDSF General Account
• NERSC HPSS Account
• NERSC AFS Account (/afs/nersc.gov)

Description (required)

Description of the work you plan to accomplish using the requested accounts.
